Attention All Athletes and Parents New Sport Registration and Clearance Procedure

We are excited to announce that the Athletic Department is launching Final Forms, an online forms system that will reduce repetitive paperwork for families. Final Forms allows you to complete and sign required athletic participation forms from your computer, tablet or mobile device. 

 Final Forms will now be used in lieu of the current sport clearance tab on the parent portal.

 Email notifications will be sent upon clearance.

Every athlete must register with Final Forms even if they currently have a valid physical on file.

 Registration for JV & Varsity Fall sports 2019 opens on Final Forms 7/28 (new private physicals due to health office 8/5).  First day of JV/V fall season is 8/19 for football and 8/26 all other fall sports.

Registration for JHS Fall sports 2019 opens on Final Forms 8/5 (new private physicals due to health office 8/19).  First day of JHS fall season for all fall sports is 9/3.

Suffolk Sports Hall of Fame Class of 2019 honorees ... 

Ward Melville Boys Lacrosse: When you think of boys’ lacrosse, you think of Ward Melville, another one of this year’s Special Recognition Award honorees. The perennial contender is the dynasty of dynasties with 10 New York State titles to its credit, including most recently in 2018 when they finished ranked as the No. 1 school in the country.
